Articular cartilage has a limited regeneration capacity. Its treatment is primarily based on stimulation of fibrocartilaginous growth, grafts and the transplantation of autologous chondrocytes. A total of 4620 articular cartilage lesions were noted in a personal series of 4463 arthroscopies. This paper examines earlier and more modern techniques. The results achieved in 49 footballers with osteochondritis dissecans (OD) or chondral defects and treated by drilling (mean follow-up9.7 yr) and microfractures (mean follow-up 17.4 months) are also presented.
