The metabolic target in elderly diabetic patients is very difficult to assess due to the lack of specific indication in this specific population. Most recently, the American Society of Geriatrics has suggested that the following parameters should be taken into account: a) individualized pharmacological and educational therapy; b) prevention of cardiovascular diseases; c) evaluation of glico-metabolic control for preventing micro-angiopathy; d) screening and treatment of major geriatric syndrome such as depression, cognitive impairment, falls and pain. It should be pointed out that only once all such a problems have been detailed, one can really evaluate the "metabolic profile" of very old diabetics.
