Introduction. Anxiety and depression in patients before and after a pacemaker implantation has been understudied despite an increased use of pacemaker therapy in Italy and in the world. The purpose of the present study was to describe and examine anxiety and depression and factors associated with them before a pacemaker implantation and within one year of the post implantation time. Methods. A descriptive, correlational and comparative design has been used to study and compare anxiety and depression in a total sample of 154 patients from two hospitals in Rome, Italy, divided in four groups: 43 patients before the pacemaker implantation, and 38, 29 and 44 patients respectively one, six and twelve months after the implantation. A Sociodemographic Questionnaire and the Hospital Anxiety and Depression Scale have been used to measure the study variables. Results. Anxiety and depression significantly decreased from the pre to the post implantation groups. In the 1-month-post-implantation group patients with a ventricular pacing had lower anxiety and depression levels than those with dual chamber pacing. In the 6-month-post-implantation group patients with a rate responsive pacing were more depressed than those without rate responsive pacing. People less old, less educated, with lower income and fewer children were more anxious and depressed before the implantation. Only older people were more depressed after the implantation. Conclusion. This study provides nurses with a first overview on the phenomenon of anxiety and depression before and after a pacemaker implantation. Knowing the level of anxiety and depression before and after a pacemaker implantation as well as factors associated with these variables helps nurses to provide patients with a better and research based care.
Introduzione. L’ansia e la depressione nei pazienti prima e dopo l’impianto di un pacemaker sono state poco studiate, nonostante l’incremento nell’utilizzo di questa terapia in Italia e nel mondo. Lo scopo di questo studio è stato di descrivere ed esaminare l’ansia e la depressione ed i fattori ad esse associati prima dell’impianto di un pacemaker e nel corso di un anno dopo l’impianto. Materiali e Metodi. È stato adottato un disegno descrittivo, correlazionale e comparativo per studiare e confrontare un campione di 154 pazienti in totale, seguiti in due ospedali romani, divisi in quattro gruppi: 43 pazienti prima dell’impianto del pacemaker, e 38, 29 e 44 pazienti rispettivamente a uno, sei e 12 mesi dopo l’impianto. Le variabili sono state misurate con un questionario socio-demografico e con la Hospital Anxiety and Depression Scale. Risultati. L’ansia e la depressione diminuivano significativamente dal gruppo pre ai gruppi post impianto. Nel gruppo ad un mese dall’impianto, i pazienti con stimolazione ventricolare avevano meno ansia e depressione rispetto a quelli con stimolazione bicamerale. Nel gruppo a 6 mesi dall’impianto, i pazienti con una stimolazione rate responsive erano più depressi rispetto a quelli senza questo tipo di stimolazione. Prima dell’impianto erano più ansiosi e depressi i pazienti giovani, quelli con bassa scolarità, basso reddito e meno figli. Dopo l’impianto solo i pazienti più anziani erano più depressi. Conclusioni. Questo studio fornisce una prima panoramica sul fenomeno dell’ansia e della depressione prima e dopo l’impianto di un pacemaker.
