Introduction: the goal of this study was to describe advanced nursing competences indicators for identification strategy in the stroke care in Italyand develop a survey tool.Problem: the new structure of the NHS and the needs expressed by patients increasingly require an advancement of the skills of health professionals.To this end the authors have identified and described a method for the detection of advanced nursing skills. Starting from the theoretical structureof AB Hamric have been identified analyzed and compared documents of a professionalizing and clinical from which have identified somecategories. For each indicator have been identified one or more items and has been developed ad hoc questionnaire. At the end this was validated.Discussion: the methodology for the identification of the indicators has been efficacy in achieving the objectives. The strategy used in the studyis reproducible, since traced to a theoretical model, and contextualized to any clinical setting, where there are secondary sources of evidencebased.It can also be adapted to post basic training course of a single reality.Conclusions: advanced clinical knowledge and skills, frequently without a formal recognition because of the complexity and instability of thepatient, are used in the stroke care. Itís hoped to use the tool to verify the effectiveness and then play back the path in other clinical setting.
Introduzione: lo scopo di questo studio è descrivere la strategia per l’identificazione di indicatori delle competenze infermieristiche avanzate nellostroke care in una realtà italiana ed elaborare uno strumento per la rilevazione.Problema: in Italia il nuovo assetto dello SSN e i bisogni espressi dall’utenza richiedono sempre più un avanzamento delle competenze deiprofessionisti della salute. A tal fine gli autori hanno identificato e descritto una metodologia per l’individuazione delle competenze infermieristicheavanzate. Partendo dalla struttura teorica di Hamric sono stati individuati, analizzati e comparati documenti a carattere professionalizzante e clinico,dai quali sono state individuate alcune categorie. Per ciascuna sono stati spiegati alcuni indicatori, per ognuno dei quali è stato sviluppato uno opiù item, successivamente raccolti in un unico questionario. Lo strumento è stato successivamente validato.Discussione: la metodica di individuazione degli indicatori si è dimostrata efficace per il raggiungimento degli obiettivi prefissati. La strategiaindividuata è riproducibile, poichè ricondotta ad un modello teorico, e contestualizzabile ad ogni ambito clinico dove ci siano fonti secondariedi evidence- based. Inoltre può essere adattata alla singola realtà.Conclusioni: attraverso questo esempio si è potuto evidenziare come nell’ambito dell’assistenza allo stroke vengano messe in campo conoscenzee competenze avanzate, spesso senza riconoscimento formale. È auspicabile l’utilizzo dello strumento per verificarne l’efficacia per poi riprodurreil percorso anche in altri ambiti clinici.
